This patient does not meet classification criteria for antiphospholipid antibody syndrome for two reasons: first, she has had only one first-trimester loss, which is a common event in healthy women. Second, the classification criteria require the presence of moderate to high levels of anticardiolipin antibodies. She does not have a lupus anticoagulant; therefore, not treating this patient is appropriate.

If she did have antiphospholipid antibody syndrome and recurrent pregnancy loss, preferred treatment includes heparin and aspirin. Both unfractionated and low-molecular-weight heparin have been used successfully. Therapeutic doses of heparin are only necessary if the woman has had a history of thrombosis. Warfarin is not appropriate during pregnancy because of its teratogenic potential
