This is an old thread but I found out no proper explanation, so I add here for someone google and end up here like me.
The answer is C
NSAIDs can cause both prerenal and intrinsic renal AKI. In this case prerenal - because its effect of AFFERENT ARTERIOLE CONSTRICTION. It will be especially exaggerated in this setting- CHF and diuretic use.

nsaids can contribute to prerenal azotemia by inhibiting renal vasodilatory prostaglandins especially in the setting of CHF
