its all blank info but the titles are there like applicants info, institution, program, program director. but nothing is filled out like not even applicants info.

You are lucky, many people did not get a single IV in Canada and as I understand all the interviews have been given out to friends and families of residents in Cananda.

if you google image NRMP match day info - the match day paper sent by NRMP to AMGs kinda looks like that.

Just tell us what each title says, exactly. That will do.

I'm pretty sure it says AAMC ID, your name, institution name

then blank blank blank

then program ID, NRMP code, etc…

This correlates with the paper people (AMGs) get on Match Day. Except the blank space is filled with "congratulations! you matched!"
