Recommendations for Vit K in INR


<5 - dec Coumadin
5-9 Stop Coumadin
9-20 Oral K
>20 IV K ( no FFP , FFP only if anything above target INR with active bleeding which could be fatal )

Lets say patient has INR of 4 and has hematuria and is on coumadin. Wud u give FFP ?

Same case with INR of 8 wht will u do ?

I tthink stop for both because inr is not dangerous level and it is not life thretaning situation may be you can give vit k for the second one

Interestingly
Pts on Vitamin K-rich diet (Green leafy vegetable's) can have difficulty attaining a therapeutic INR, thus, they require higher doses of Warfarin to acheive the desired INR.


I am sure you all know
