Well, uremia is pathological manifestations of severe azotemia.

Azotemia is one of many clinical characteristics of uremia, which is a syndrome characteristic of renal disease. Azotemia is marked by increase of nitrogenous waste and is classified in relation to the source of kidney damage i.e prerenal, renal, post renal.

Uremia includes azotemia, as well as acidosis, hyperkalemia, hypertension, anemia and hypocalcemia along with other findings. And GFR ia about 10ml/min. Usually seen in chronic kidney diseases and in liver failure.

Hope Ive cleared the air.
