Give liberal IV fluids like at least 3 lters of normal saline, then check urine output, but usually give at least 200 ml per hour, then give insulin at 8 to 10 units per hour, then when glucose is < 250-300 ml combine insulin treatment with D5W . Continue until acidosis is resolved, remenber there is a greatly increased insulin resistance during DKA .
Bicarbonate is given only if PH < 7.0 TO 7.1 ESPECIALLY IF PTS IS HAVING RESPIRATORY OR HEMODYNAMIC COLLAPSE, if the K is normal or low at the start of the treatment give KCL.
