A 65-year-old Laotian immigrant has developed thick, erythematous nodules on her ears and nose with significant associated sensory loss. The nodules have grown slowly over the course of many years. Biopsy of the lesions shows dermal granulomas with giant cells but no acid-fast bacteria. Culture on blood agar and Lowenstein-Jensen medium shows no growth. These findings are most consistent with which of the following diagnoses?

A. Cutaneous leishmaniasis

B. Onchocerciasis

C. Rhinoscleroma

D. Smallpox

E. Tuberculoid leprosy

i know its e...

in tuberculoid lepromatosy you have a nice TH1 response... granulomas.

in lepromatose... not a good response.

i got confused for a second with the fact that it affected the nerves... which could also hapen in A... but well.. it happens in lepra too.. so dont mind my post.

:O)

the question is just asking about the TH1 response in tuberculoid.
