Pemphigus vulgaris
Immunologic disease with IgG antibodies directed against the intercellular attachment sites (desmosomes) between keratinocytes (type II cytotoxic antibody hypersensitivity reaction); produces vesicles (blisters < 5 mm) and bullae on skin and within oral mucosa; vesicles are intraepidermal and have a suprabasal location (just above the basal cell layer), leaving basal cells intact and looking like a row of tombstones; due to detachment of individual keratinocytes from each other (acantholysis), keratinocytes are present within vesicle fluid; bullae exhibit Nikolsky's sign (outer epidermis separates easily from the basal layer with minimal manual pressure)

Bullous pemphigoid
Immunologic vesicular disease (IgG antiepithelial antibodies directed against the basement membrane; type II hypersensitivity) in which vesicles are in a subepidermal location; Nikolsky's sign is negative; acantholysis is not present
