defirillation is based on rhythm on defibrillator, a pt is never defibrillated without knowing the rhythm.
witnessed or unwitnessed , if u dont know the rhythm , its ALWAYS and ALWAYS CPR. Cardiac arrtest could have two basic rhythms - Ventricular fibrillation and asystole. witnessed CA == and the rhythm is a vent fibr( if the pt was in CCU where he will be connected to a monitor) u can try a cardiac thump, only once. never thump an asystole. unwitnssed CA == always start with CPR (unless u can get a defirillator to monitor the rhythm even before u can start CPR, usuaaly not possible) once u get the monitor, IF rhythm, VF- shock(defibrillation),if rhythm asystole continue CPR, epinephrine helps in sensitising the heart. |
