when a sp is siting on chair that means no physical examination he might be there for insurance form fill or drug refill only in that case no need to do physical examinations , if the Spcomes with complaints tell him at the end the list of physical examinations we are supposed to do in future and in that case no need to mention anything in patient notes can be left blank
