thiazide diuretics - torsades

causes hyper uricemia and predisposes to gout ... can anyone plz explain the mechanism??

-main mech by which thiazides cause hyperuricemia is due to hypovolemia.
-urate handling occurs in PCT.
-hypovolemia induces excess Na resorption in PCT-----> Na/H exchanger throws out H+ -----> increased OH- ions in cell tend to follow H+ outward, and Urate is absorbed in place of OH-.

low-dose thiazides or maintaining normovolemia minimizes this effect.
