Common conditions tested are: a Fractured Scaphoid causing pain on the radial side on palpation of the "anatomical snuff box." DeQuervain's tenosynovitis (with a positive Finkelstein's test) due to inflammation of the extensor pollicus brevis, and abductur pollicus longus, and Triangular Fibricartilage Complex (TFCC) tear causing pain on the ulnar side of the wrist. While carpal tunnel syndrome can cause wrist pain, it more likely causes numbness in the median nerve distribution (thumb, 2nd, 3rd, 1/2 of 4th fingers on the volar (palm) side of the hand) and awakens patients from sleep. Inspect it forobvious deformities (most common is RA ulnar deviation and Colles' Fracture with trauma) and check active range of motion. Palpate for warmth or effusion (seen usually on the dorsum of the wrist). Then,
For carpal tunnel - check Tinel's test by tapping the mid-volar aspect of the wrist and asking about it causing pain or numbness in the hand; Phalen's sign - flex the wrist 90 degrees for 30 seconds and ask if it reproduces the pain or numbness
For TFCC tear - test pain with ulnar deviation and mild wrist compression toward the ulna
For DeQuervain's synovitis - cross the thumb over the palm, close 4 fingers over the thumb, and and ulnar deviate the wrist (Finkelstein's test). Pain produced is a positive test.
For Ganglion syct - Flex and extend wrist and watch for a nodule protruding from the wrist
For fractured scaphoid - fully extend the thumb and palpate in the depression between the 2 tendons visible over the space between the thumb and wrist
