ya appendix is the most co site but when it comes to carcinoid tumors leading to carcinoid syndrome ileum is the most common cause

I remember this Q. I made the same mistake. But I was thinking even worst by selecting lung cancer as my ans..
I knew like everybody that appendix is the MC site overall. But the patient was symptomatic in this Q, and somehow I couldn't get rid of the thought that lung carcinoma are more likely to be symptomatic (better prognosis), as serotonin produced by lung by passes liver and don't get metabolize. Where as for intestinal carcinoids to become symptomatic they must wear out the capacity of liver to metabolize Serotonin. This could take time, but by the time it shows its symptoms it's already metastasized ( bad Q ) Do you think my read on this Q is legit or is there any better explanation to this Q?

Carcinoid syndrome (diarrhea --> flushing) (or as i like to say..."everything comes out") is ONLY when it metastasizes....how? TERMINAL ILLEUM ----> LIVER with invasion and metastasis they can be in the foregut (stomach) Midgut (Terminal Illeum)
