I replied to this qs before and this is the answer:
first of all I will talk about IM only..... prelim in IM is totally different from other specialties. when u apply to prelim, this means that u want to apply to one of these specialties in the next year ( ophthalmology - dermatology - neurology - urology , and some others) those specialties are very very competitive in the US, and ur chances to get this prelim is so hard, just like applying to specialties like I mentioned above. so apply to categorical only........because if u applied both, u will be asked what specialty u want to be in the next year, but in real u want IM nothing else, so u will be in very difficult situation to explain ur concerns in this mess.
