drug of choice for shock, hypotensive, angioedema, airway obstruction, bronchospasm, and urticaria in severe anaphylactic reactions----> IM epinephrine is better for better absorption and more rapid than sq.

In UW MCQ mentioned IM Epinephrin and in UW CCS IV Epi for sever case..........

in severe case, u r right, IV epinephrine but If an intravenous line cannot be established, the intramuscular dose can be injected into the posterior one third of the sublingual area

If patient is in anaphylactic 'shock (see below, severe hypotension), IV epi should be considered.

If an intravenous line cannot be established, the intramuscular dose can be injected into the posterior one third of the sublingual area, (as sami mentioned) or the intravenous dose may be injected into an endotracheal tube.
