Yes, if you have done one year of residency in the US, and passed all your steps, you qualify for an unrestricted medical license in Puerto Rico and can even sign up to accept insurance plans as a "generalist." I would imagine there are employers there who will sponsor a visa. But again, to be effective in your job you will need to speak fluent Spanish.
