in emergency do I need to consult and write the procedure name at the same time e.g following are at the same time in ectopic pregnancyat, foeign body aspiration but in other cases consult result is first received then order procedure eg sigmoid volvulus, ruptured AAA,perforated duodenal ulcer, cholecystitis, appendicits.
archer told that to get consult recommendation first then write down the procedure name.archer even told that if surgeon say no to surgery or procedure then DO NOT WRITE THE PROCEDURE AT ALL,,,it means we always need to have consult result before writing procedure...but that thing can take time in emergency and do surgeons also refuse procedures then should we go for that procedure????
if its clinical diagnosis then can I write the procedure name and consult at the same time??????????? but again consult result, may b surgeon will say NO

mle3mle, you need to see feedback from consultant and if you met the criteria for surgery, consultant message will pop up " patient will be scheduled for surgery." . If such a message did not pop up it means either you did not meet indication for surgery because of poor sequencing of orders or surgery may not be needed in that case. Doing invasive procedure when it is not indicated huts the whole case badly and fails it.
Archer explains this clearly by dissection of Aorta case. When you put bad sequencing and order surgery consult before ordering a CT scans, surgeon says " NO" . Where as you order surgery consult after CT Scan , surgeon accepts consult. Ct scans are needed to show ascending thoracic aorta disssection before surgery indication is met - surgery is indicated in ascending not in descending dissection. We should have that info before we call surgeon . Follow exactly what Archer ccs says because UW does not give clear idea on all these very important points.
