A 66 y/o M present to you with complains of difficulty in swallowing some hoarseness of his voice, and giddiness that started 96hrs earlier. On PE, you found that pat loss the pharyngeals gagging reflex, L facial analgesia, and L paralysis vocal cord.

Dx?
a) Lateral medullary syndrome on L side
b) Lateral medullary syndrome on R side
c) Medial medullary syndrome on R side
d) Medial medullary syndrome on L side
e) A meningeal tumor in poster in the posterior cranial fossa on R side

1)this is PICA, Lateral Medullary sdr.(Wallenberg)
2) the qs is now is it L side or R side?
3) I think is Left side Wallenberg bc IPSIlateral face analgesia
4) to add more ...it could present w Contralateral body pain& T. probl. & IPSI Horner
