A 59 y/o M just was released from rehab. 2nd day he got the news that wife of 40yrs left him for another woman (hard news). He goes back to drinking again, his taking Acetaminophen bz his friend PA recommended for control muscle pain he suffers from stress. He starts having nausea and vomiting and pain RUQ. He rushed to ER.
Treatment?
a)Aspirin
b)Ethanol
c)Methanol
d)Mercpatan
e)Vitamin A
f)Vitamin C
g)Copper

I dont get this.
patient has Acetaminophen toxicity, right?
and the fact that hes an alcoholic makes these patients even more susceptible to toxicity
wouldnt you give NAC to replenish glutathione?

Pat that I created here w pain/wife/alcohol/ suffer from Acetaminophen tox.
- Microsomal ethanol oxide sysm he is alcoholic [ CY2E1] as u know will convert Tylenol -> toxic.
Tylenol ------------------EtOH + --------------------> NAPQI ( that is toxic)
Now NAPQI -----------Glutathione/S-transferase ( that is where u add N-acetyl cysteine) to convert->
N-acetyl-cysteinyl derivative (mercapturic acid) - > and pee it out. And help pat here.
Glutathione is a mercaptan ( w a free sulfhydryl)
U all know we give N-acetyl cysteine as INC glutathione.
DONT FORGET ALL MY Qs are timed. Either u know it OR you don't. No searching on my Q.
