I've not been having much success studying on my own (low 200s on my NBMEs but need 250+, have been studying for 4 months dedicated already)
Can anyone recommend a good online Step 1 tutor (am home in Italy studying)? A friend recommended med school tutors but their rates are really high like $250 an hour which is crazy to me...

Hey, I have a small tutoring company and we charge significantly lower rates than MST but have higher scoring tutors (we pay them more). I personally scored a 262 on Step 1 and 281 on Step 2 CK, and my tutors are all very experienced and similarly high scoring.
