Is H visa is ok for spouse -

Hello
Me and my wife want to apply for residencies that sponsor H visa. My question is:
As far as I know someone has to get a ECFMG certificate and than pass Step 3 to apply for a H visa residency spot.

So, should we both pass through this process? If only one of us pass the step 3 and get the H visa eligibility, is the other one also become eligible for H visa automatically.

One of you should have USMLE 3 to apply for H1B visa and the ther spouse simply will follow with H4 as dependent and shall not be eligible to work and if both write USMLE 3 both of you will get H1B independent of each other's visa status.
