DoctaM3

Many many thanks for all your support during the revision period!
This is by the far the toughest exam I have done to date.
Your revision plan is spot on and I followed it as much as I could.
Good luck in your residency.
A great doctor is one who helps others and shares his/her medical knowledge.
Please clarify..

In UW CCS 1st case is UTI with pregnancy..

Let's say its 18 min case in exam ( which is actually 18 min case in UW), is that ok if we order all 1st trimester prenatal lab test (Rh, Hep B , HIV and all) on her first visit though she is in office with UTI complaints ??? Because UW has simply treated her for UTI and just ordered prenatal vitamins and prenatal counseling. But 18 min time is long.. What do you think??

In the CCS time is VERY TIGHT and the key to passing well is to score all the points for that case i.e. correct investigations, treat the UTI and appropriate follow up.

You can order whatever you like but in the exam you will run out of time and not complete the case.

The secret is to put in the basic and correct orders and then forward the clock as fast as you can. You must get to the end of the case. When I was preparing and doing timed CCS cases I kept running out of time again and again. Keep practicing and get faster and faster.

18 minutes is not too long...you will see. Most cases will end early if your management is correct otherwise you will be going around in circles with the software.

Keep going it will soon make sense.

PS In the real exam I only got the final diagnosis screen twice in the 12 cases. Good luck
Thank you jsd, your words are helpful.. I will follow that..

yesterday I was doing above case, just finding UA positive, hcg positive and treating her with antibiotics took less than 10 min..Alloted time was 18 min.. So I thought they may also expect detail prenatal tests.. Archer also says in lecture that if you got 18 min pregnancy case, then you have to order detail 1st trimester test and if its 10 min case, then manage only chief complaints for which pt has visited..But in UW it was 18 min case and didn't show any prenatal 1st trimester tests..

Thank you.

As per archer you need to address all her issues, it is an office case u have 18 min you need to provide 1st trimester care and investigations. Add relevant labs and Folic acid.
regardless of time on "normal" uncomplicated pregnancy case.. if the pt. presents in 1st trimester...there are a total of about 16 orders that you need to do for 1st trimester. practice practice practice in typing them out one after the other without pausing to think about them (esp. if its a 10 min case)

@doctam...
If I'm not mistaken, you should have started residency by now???
What's this special attachment to usmle forum????
You are even annoyingly acting as mr know all moderator of the forum! Pls grab a life
special attachment is the want to help others succeed and get on with their lives because people did that for me previously on here. its called paying it forward.
